Cardiovascular collapse induced by position-dependent pulmonary vein occlusion in a patient with fibrosing mediastinitis.

چکیده
FIBROSING mediastinitis is a rare disease characterized by an abnormal proliferation of fibrous tissue and acellular collagen within the mediastinum. Most cases of fibrosing mediastinitis in the United States are caused by an exaggerated granulomatous immunologic response to Histoplasma capsulatum infection. However, other possible etiologic conditions include tuberculosis, mediastinal malignancy, and autoimmune diseases. In advanced stages of the disease, these patients may have signs and symptoms of extrinsic compression of airways, intrathoracic vessels, or the esophagus. We describe a patient with fibrosing mediastinitis who, after sedation, awake tracheal intubation, and being positioned supine, experienced acute cardiovascular collapse due to occlusions of the pulmonary veins. It is well known that patients with anterior mediastinal mass can experience posture-dependent compression of airways, vascular structures, or both. However, our case is the first report of position-dependent occlusion of pulmonary venous outflow associated with a disease that is primarily distributed throughout the middle mediastinal compartment.

Thoracoscopic pneumonectomy in management of histoplasmosis and fibrosing mediastinitis.
Pulmonary histoplasmosis is generally a self-limited respiratory illness in endemic areas. Fibrosing mediastinitis is a severe chronic complication of pulmonary histoplasmosis in which pulmonary vessels and airways can be compressed with the potential for life-threatening implications. We present a 50-year-old male patient who presented with a total occlusion of the left pulmonary artery due to...
متن کاملPulmonary vein thrombosis after metastatic liposarcoma.
Pulmonary vein thrombosis has been described in several patients including those with malignancy, pulmonary veno-occlusive disease, fibrosing mediastinitis, and after lung transplantation. 1-5 Although liposarcoma frequently metastasizes to the lungs, to our knowledge pulmonary venous (tumor) thrombosis with lung infarction has not been previously mentioned as part of this disease. We report a ...
متن کاملPulmonary vascular occlusion and fibrosing mediastinitis.
Patients with fibrosing mediastinitis causing obstruction of pulmonary veins and arteries may present with many of the historic, physical, and laboratory findings of patients with pulmonary hypertension due to chronic thrombotic obstruction of major pulmonary arteries. Because the latter is subject to surgical correction, and the former is not, it is important to differentiate between the two a...
متن کاملFibrosing mediastinitis with pulmonary hypertension as a complication of pulmonary vein stenosis
INTRODUCTION Fibrosingmediastinitis (FM) is caused by a proliferation of fibrous tissue in the mediastinum encasing the mediastinal viscera that results in compression of mediastinal bronchovascular structures. Pulmonary hypertension (PH) is a severe complication of FM caused by extrinsic compression of pulmonary blood vessels. CASE PRESENTATION Here, we present the case of a 47-year-old man ...

Fibrosing mediastinitis (FM), also known as sclerosing mediastinitis, is an uncommon but serious disease involving the mediastinal structures. A high index of suspicion is essential to establish the diagnosis of FM and starting the appropriate therapy for patients. Here, we report a case of a young female who presented with chest symptoms and subsequently underwent different laboratory and radi...
